Reduce Preventable Readmissions

The reduction of patient readmissions to acute care hospitals has become a national patient safety focus. In April 2011, the Obama Administration launched a new project called "Partnership for Patients: Better Care, Lower Costs". The project seeks to make hospital care safer, more reliable, and less costly. Two specific goals have been identified for the project:

  • Keep patients from getting injured or sicker, by reducing preventable hospital-acquired conditions (HACs)by 40% by the end of 2013.
  • Help patients heal without complication, by reducing preventable hospital readmissions by 20% by the end of 2013.

The PIN is conducting this clinical improvement study in support of the national goal to reduce preventable hospital readmissions, specifically among its CAH members.
